NEW PATIENT HEALTH EVALUATION - $250

During this health evaluation, you will have the opportunity to spend 90 minutes with the doctor discussing your health concerns. The doctor will collect a thorough medical history, conduct physical exams, order necessary lab work and/or radiologic imaging to aid in treatment decisions. The tests that will be run on ALL new patients are included in the Basic Evaluation Blood Panel.

If you have had bloodwork run in the past 4 weeks, please bring them to your appointment. The doctor will evaluate the results and determine what additional workup, if any, is needed to determine the best treatment plan.

All new patient appointments require a non-refundable $60 deposit to schedule. You may reschedule your new patient evaluation 1 time, without penalty.
ORTHOPEDIC HEALTH EVALUATION - $100

During this health evaluation, you will have the opportunity to discuss the specifics of your injury. Our doctors have extensive experience in diagnosing and treating a variety of muscle, ligament, tendon, and joint concerns.

ALL new patients with orthopedic injuries will receive an orthopedic evaluation which includes history of injury, physical exam, and ordering of imaging/bloodwork.

If you are an existing patient with a new injury, it is necessary to have an Orthopedic Health Evaluation to determine proper treatment.

Continuing Care Evaluation (10 minutes) - $50
Office or other outpatient visit for the evaluation and management of an established patient, which requires at least 2 of these 3 key components:

1) A problem focused history; A problem focused examination; Straightforward medical decision making.

2) Counseling and/or coordination of care with other physicians, other qualified health care professionals, or agencies are provided consistent with the nature of the problem(s) and the patient's and/or family's needs.

3) Usually, the presenting problem(s) are self limited or minor.

This is time spent face-to-face with the doctor.

Continuing Care Evaluation (15 minutes) - $75
Office or other outpatient visit for the evaluation and management of an established patient, which requires at least 2 of these 3 key components:

1) An expanded problem focused history; An expanded problem focused examination; Medical decision making of low complexity.

2) Counseling and coordination of care with other physicians, other qualified health care professionals, or agencies are provided consistent with the nature of the problem(s) and the patient's and/or family's needs.

3) Usually, the presenting problem(s) are of low to moderate severity.

This is time spent face-to-face with the doctor.

Continuing Care Evaluation (30 minutes) - $100
Office or other outpatient visit for the evaluation and management of an established patient, which requires at least 2 of these 3 key components:

1) A detailed history; A detailed examination; Medical decision making of moderate complexity.

2) Counseling and/or coordination of care with other physicians, other qualified health care professionals, or agencies are provided consistent with the nature of the problem(s) and the patient's and/or family's needs.

3)Usually, the presenting problem(s) are of moderate to high severity.

This is time spent face-to-face with the doctor.

Continuing Care Evaluation (45 minutes) - $150
Office or other outpatient visit for the evaluation and management of an established patient, which requires at least 2 of these 3 key components:

1) A comprehensive history; A comprehensive examination; Medical decision making of high complexity.

2) Counseling and/or coordination of care with other physicians, other qualified health care professionals, or agencies are provided consistent with the nature of the problem(s) and the patient's and/or family's needs.

3) Usually, the presenting problem(s) are of moderate to high severity.

This is time spent face-to-face with the doctor.

Continuing Care Evaluation (60 minutes) - $200
Office or other outpatient visit for the evaluation and management of an established patient, which requires at least 2 of these 3 key components:

1) A comprehensive history; A comprehensive examination; Medical decision making of high complexity.

2) Counseling and/or coordination of care with other physicians, other qualified health care professionals, or agencies are provided consistent with the nature of the problem(s) and the patient's and/or family's needs.

3) Usually, the presenting problem(s) are of moderate to high severity.

This is time spent face-to-face with the doctor.

BASIC HORMONE EVALUATION PANEL (Female) - $445

We provide a cash pay option for patients who prefer to pay out of pocket for blood work.
